Spinal block is an injection of an anesthetic into strictly defined points located along the spinal column. Often it is not a method of treating an existing disease and does not have any therapeutic effect, but it can quickly relieve the pain it causes. Although there are therapeutic blockades that involve the administration of corticosteroids.

The procedure is performed exclusively in neurosurgical or traumatology medical institutions by qualified medical personnel. No nurse or doctor who has not undergone special training has the right to perform a spinal block, as this is fraught with serious complications and loss of the ability to move.

With “SL Clinic” you no longer need to look for where to perform a back block. By contacting us, you won’t have to wait long in line to relieve a severe attack of pain. Highly qualified specialists of “SL Clinic” will immediately perform all the necessary manipulations with strict adherence to all the subtleties of the blockade injection technique. The price of a spinal block is indicated in the price list and remains consistently affordable for a wide range of people.

Pain levels

0 — 3

Mild pain

- does not interfere with everyday activities, but you can adapt to the pain.

4-6

Moderate pain

- prevents you from doing business, prevents you from concentrating, the patient remains independent

7-8

Strong pain

- it is impossible to carry out daily activities, it is difficult to think about anything other than pain, the patient cannot function independently

9-10

Unbearable pain

- requires urgent care, bed rest, disables the patient

Why is there such severe pain?

  • One of the reasons may be muscle spasm and pinching of the sciatic nerve, which may cause pain in the lower extremities or their numbness;
  • A hernia can cause sciatica - this is a pinching of the so-called cauda equina - nerve fibers that are woven together. Due to the pressure on them, severe pain occurs;
  • The hernia can also put pressure on the nerve fibers passing through the lateral openings of the vertebrae. In this case, the pain is constant and very strong, often radiating to the upper and lower extremities.

In most cases, specialists do not perform operations immediately, but try to eliminate the cause and symptoms with the help of medications and special physical exercises. For the treatment process to be effective, the patient is given a spinal block, which suppresses pain.

Blockade of a spinal disc herniation: benefits

Drug blockade is the administration of a drug to the site of compression of the nerve root in order to quickly relieve pain. The most important advantage of blockades is the speed of the drug’s effect on the body. Usually the patient feels relief immediately after the procedure, which lasts for a long time. If necessary, the procedure can be repeated several times - sometimes one is enough, and sometimes you have to do a course of 5-7 blockades.

The blockade also helps relieve inflammation and swelling of the nerve root, normalizes blood circulation and eliminates painful muscle spasms in the back.

To obtain the best effect, the drug is injected into 2,3 or 4 painful points, sometimes on both sides of the spine.

Feedback on effectiveness

Doctors and patients confirm that when carrying out a blockade, the effect occurs very quickly . Patients immediately forget about the pain. True, in some cases it is difficult to achieve complete pain relief. Several procedures are required. Reviews also confirm that complications after this manipulation are extremely rare.

But blockades are a temporary procedure . Typically, pain returns 3-4 weeks after a paravertebral block or several months after an epidural.

What kind of blockades are used for vertebral hernia?

Depending on the injection site, two types can be distinguished:

  • Paravertebral - the medicine is injected into the area where the nerve passes under the transverse process of the vertebra.
  • Transforaminal - the medicine is injected deeper into the intervertebral foramen to the site of root formation in the spinal cord.

Depending on the drugs used during the procedure, we can distinguish:

  • Anti-inflammatory

    . For example, when blocking an intervertebral hernia, Diprospan is used, a drug with powerful anti-inflammatory properties.

  • Painkillers

    . Novocaine spinal blockade or lidocaine blockade for spinal hernia, a local anesthetic instantly blocks the transmission of pain impulses along the nerve.

  • Mixed blockades

    - used most often. The injection includes 2-3 components - anti-inflammatory + painkillers, this “cocktail” allows you to get the greatest effect from the blockade.

Paravertebral blockade is most often used.

Preparations for performing spinal blockade

Multicomponent formulations are often used, the basis of which is still known anesthetics - lidocaine and novocaine. They block the transmission of nerve impulses by inhibiting sodium channels.

Also, solutions for blockade injections may include:

  • corticosteroids are powerful anti-inflammatory drugs, the delivery of which directly to the site of inflammation helps to quickly eliminate swelling and severe inflammation;
  • B vitamins – help normalize the transmission of nerve impulses;
  • chondroprotectors – necessary for the active saturation of cartilage tissue, restoring its elasticity and strength.

Injections for a hernia in the back: Blockade technique

The paravertebral block procedure is performed with the patient lying on his stomach. The points of greatest pain on both sides of the spine are determined. Having retreated 3-4 cm from the line of the spinous processes of the vertebrae (central line), after preliminary anesthesia of the skin, the medicine is injected into the area of the inflamed nerve to a depth of 5-7 cm. On average, to achieve the desired result, it may take from 3 to 7 injections, with an interval of 1 to 7 days. The effect after paravertebral blockade can last up to several weeks. It is recommended to use it in combination with other treatment methods.

Orthopedic surgeon Andrey Yurievich Sidenkov comments:

Epidural anesthesia is carried out following the same steps, only the injection is made deeper, piercing the hard shell of the spinal cord. This blockade is done in a hospital setting. A similar technique is used for injections for hernia of the cervical spine.

Possible alternative

Thus, blockades are an effective way to relieve pain. But they do not solve the problem, but only temporarily remove its symptoms. Yes, and they can only be used 4 times a year, and in addition, they have a large number of contraindications.

To completely eliminate the problem, it is necessary to influence the causes of pain. Since they are most often provoked by pinching of the nerve roots extending from the spinal cord, the causes should be sought in the spine.

The quality of work of all components of the body largely depends on the condition of the spine. Manual therapy and especially the Gritsenko method can correct age-related changes in it, restore the normal position of the vertebrae, correct defects in posture and eliminate pressure on the nerve endings. This method of treatment directly affects the causes of pain and helps reduce their frequency and intensity. At the same time, manual therapy is absolutely safe and has a very narrow list of contraindications.

Indications

Medical research proves that any acute pain in the back, especially the lower back and sacrum, should be quickly relieved. Otherwise, they can provoke the development of psychological disorders, which will worsen the effectiveness of the treatment.

Also, with a long course of the disease, a persistent focus of excitation can form in the cerebral cortex. Any additional irritation can cause its activation and the development of a new attack. In such situations, it is very difficult to improve the patient's condition using conservative methods. Therefore, it is much better to prevent the formation of a focus of excitation in the cerebral cortex. If conventional means do not help eliminate pain, a spinal block is used.

The method is intended to relieve pain caused by:

  • intervertebral hernias;
  • protrusion of intervertebral discs;
  • myositis;
  • radiculitis;
  • neuritis;
  • ankylosing spondylitis;
  • osteochondrosis;
  • spondyloarthrosis;
  • intercostal neuralgia;
  • trigeminal neuralgia;
  • neuralgia of the spinal nerves.

It is worth giving a blockade injection no more than 4 times a year. Although in certain cases, patients are prescribed a course of such injections, consisting of 10 or more procedures. Between them there is a break of 2-3 days.

The method is also often used to diagnose a number of diseases. The effectiveness of the blockade for pain indicates the neurological causes of its occurrence. Otherwise, you should look for another source of pain.

Contraindications to the introduction of a blockade

There are also contraindications to setting up a blockade. This procedure involves a small tear of tissue, and if a person does not have rapid blood clotting, then there is a risk that the medicine will enter the spinal cord and cause complications. Also a serious contraindication is excessive sensitivity to certain drugs. The patient may experience anaphylactic shock, which can even lead to death.

The blockade procedure has some contraindications

The most common contraindications include:

  • Hemophilia, pathology of the circulatory system.
  • To whom.
  • Presence of infectious diseases.
  • Epilepsy.
  • Heart problems, as it may not withstand the effects of analgesic drugs.
  • Pregnancy.
  • Liver lesions.
  • Vascular hypotonia.
  • Myasthenia.
  • Mental illnesses.

Remember! The procedure should only be performed by a doctor. Under no circumstances should you try to administer the blockade yourself, even if you know how to do it.

Execution technique

The patient lies on the couch; the position taken should not cause pain. The anesthesiologist determines the injection site. In general, several injections are given in different places. The effect of anesthetic on the affected area helps eliminate spasm and pain. The injection lasts for about a month. In some patients, the pain does not go away completely, but only subsides. In such cases, a course of treatment is required.

Immediately before the invasion, the injection site is treated with an antiseptic solution, the manipulation is carried out using a thin, long sterile needle. Anesthesia is given on both sides of the vertebra. Usually, lidocaine, novocaine, or a combination of the latter with hydrocortisone is taken for the blockade. The doctor’s task is to administer injections in increments of 1.5 cm along the spinal column. The depth of needle insertion is 2–3 centimeters.

The procedure is not the most pleasant, but the patient should try not to move. To reduce discomfort, the injection site is pre-treated with an anesthetic. The needle is first inserted all the way, then partially removed and directed towards the process by about 2 cm. The optimal number of invasions is six points on each side of the spine. The manipulation is carried out under the control of equipment, using x-ray data.

When performed correctly, the positive effect occurs very quickly: muscle spasm is eliminated, signs of inflammation disappear, and mobility improves.

The procedure is carried out using a special long needle.
