What kind of massage for a fracture of the surgical neck of the shoulder, we consider in detail

Shoulder fractures are a common occurrence in trauma practice. This injury usually occurs as a result of the application of a large force that the bone tissue is simply unable to withstand.

In order for the patient to quickly recover in the future and his musculoskeletal system to return to normal, it is recommended to do a special therapeutic massage after a shoulder fracture.

This physiotherapy promotes rapid restoration of bone tissue, providing a strengthening effect.

Symptoms

Before massaging the shoulder joint after a fracture, you should first make sure that this procedure will not harm. This requires an examination (X-ray), and the diagnosis must be made by a traumatologist. But the main symptoms are also taken into account.

11

Signs include:

  • shoulder shortening;
  • pain in the area of injury;
  • there may be bruising and swelling at the site of injury;
  • if there is a displaced fracture, then deformation of the shoulder may be observed;
  • difficulty moving the joint;
  • When palpating the area of injury, cracking or crunching sounds may occur from bone fragments.

Sometimes with impacted traumatic injuries, when a broken piece of bone is driven into another, a strong fixation can occur.

During this damage, pain and other unpleasant symptoms are mild; for this reason, the patient may simply not notice it for a long time. It is very important to be attentive to your body, undergo a timely examination and consult a doctor.

Treatment periods and their duration

Exercises for the shoulder joint after a fracture are selected depending on the period of treatment, and at the first stage their choice, of course, also depends on the method of immobilization of bone fragments.

Type of fracture and main method of fixationAverage duration of treatment periods
Fractures of the acromial end of the clavicleThe abduction splint (angle at the shoulder joint is 90°) is removed after 30-40 days. The duration of the second stage of treatment is purely individual, and for complete rehabilitation of the acromioclavicular joint it will take from 6 to 8 months from the moment of injury.
Fractures of the body of the scapulaFor injuries to the body of the shoulder blade, wear a Deso bandage for 10 days (pictured on the left). Then it is replaced with a supporting scarf, leaving the bend angle at the elbow joint equal to 90°. If the neck of the scapula is broken, the limb is fixed with an abduction splint. In both cases, the fixation is canceled after 30 days. The second period of functional recovery lasts the same amount. Full recovery will take 2.5-3 months.
Fractures of the upper end of the humerusFor impacted and adduction types of fractures of the surgical neck of the humerus, a Dreving support bandage is put on (elbow bend angle - 35°), placing a roller in the armpit, and for abduction types - an abduction splint. Immobilization lasts 3-5 weeks. Restoration of hand functions, subject to daily exercise therapy, occurs approximately 45-60 days after injury.
Fractures of the body (diaphysis) of the humerusIf fragments of the body of the humerus are displaced, osteosynthesis can be performed first, but in any case, the broken arm is placed on an abduction splint for 30-35 days. Exceptions are cases when the Ilizarov apparatus is used. Recovery is possible 60-90 days after the fracture, and full sports are allowed after 120 days.

Attention! For fractures that occur in the lower third of the humerus, including the condyles and epicondyles, a set of exercises is compiled according to the method used in the treatment of injuries to the elbow joint.

General rules

The therapeutic effect of exercises is based on systematicity and gradual complication

For exercises performed after damage to the shoulder joint or diaphysis of the humerus, there are general rules for their execution:

  1. To achieve a therapeutic effect, exercises are performed daily, comprehensively, and several times a day. During class, if necessary, you can take pauses between exercises to rest.
  2. You cannot exercise if the patient has an elevated body temperature above 38° or there is severe pain, not only in the area of injury, but also in any other place.
  3. In addition to special movements for a broken limb, the complex includes breathing and general developmental exercises. Breathing exercises are required to be performed while wearing any fixation device or product. This is due to the need to enhance ventilation of all parts of the lung on the affected side, which is impaired by inevitable stiffness, restriction of movement of the ribs and shoulder joint. After immobilization is lifted, breathing exercises may not be performed.
  4. In the complex, exercises should not only alternate correctly with each other - dynamic with static, movements for small muscle groups with movements for large muscles, strength exercises with exercises for stretching muscle fibers and joint mobility - but also be performed while maintaining the optimal rhythm of movements, taking into account type of injury and period of treatment. It has been proven that the correctly selected rhythm of the exercise reduces the load on the nerve endings and delays the onset of fatigue.

Help callus formation by normalizing drinking balance and special diet

During the immobilization period, a complex of exercise therapy is done 8 times a day, with equal intervals of time between them.

In this period:

  • exercises begin to be performed with a minimum number of repetitions, usually 6 times;
  • it is necessary to increase the load up to 12 repetitions gradually, for example, with a fracture of the diaphysis, the first period of treatment will take at least 28 days, which means that in the first week 6 repetitions are done, in the second 8, in the third 10, and in the fourth and further 12 times;
  • exercises are done slowly at first, and then at an average pace;
  • the range of motion is specified separately for each exercise;
  • twitching and jerking are unacceptable;
  • The duration of one session is from 15 to 20 minutes, with a single peak of increasing the load on the broken limb.

In the second period of treatment, after immobilization is lifted, exercises to restore the shoulder after a fracture should also not tire the patient, but only cause him a feeling of cheerfulness, a surge of strength and lift his spirits. Each of the exercises, unless otherwise specified, is repeated 10 to 16 times.

The number of movements in a lesson, their complexity and amplitude increases, and “training” is carried out 4-6 times a day, for 30-40 minutes, with 2 peaks of load density, approximately from 10 to 15 and from 25 to 35 minutes.

Therapeutic dosed swimming is the best form of rehabilitation after fractures

The bulk of different vectors, performed independently, without the help of a healthy arm or auxiliary objects, active movements in the joint during a shoulder fracture are done in the third (training) period.

It is recommended to plan it like this:

  1. 20-minute morning exercises, in which about 12-15 minutes should be allocated for exercises for a broken arm.
  2. If possible, a 45-minute session of measured swimming, 2-3 times a week. In case of fractures of the shoulder joint and/or humerus, jumping from a platform, starting from a stand and swimming butterfly is prohibited for 6 months from the date of injury.
  3. A 60-minute complex of special exercises for the injured limb, including exercises that would simultaneously strengthen not only the arms, but also other muscle groups or increase the range of motion in other joints. The training should have 3 peaks of load density on the broken arm - from 15 to 20, from 30 to 35, from 45 to 50 minutes. It is best to exercise in the evening, so that there is a gap of 30 minutes between the end of the workout and the start of dinner. If it is not possible to visit the pool, then this complex should be performed 6-7 times a week. When visiting the pool, a set of exercises after a fracture of the humerus is either performed at the opposite time of day or not done at all.
  4. A 10-minute evening cool-down before bed from yoga asanas.

On a note. It should be understood that no means can be used to accelerate the formation of callus and its transformation into bone tissue. But a lack of water, minerals and vitamins, as well as the absence or, conversely, excessive loads can slow down the healing process and the time until the final rehabilitation of a broken shoulder.

Features of massage

Massage for a fractured humerus is an important step in the rehabilitation period. This procedure is recommended to be carried out in several courses, which should consist of at least 10 sessions. The number of procedures may depend on the type and complexity of the fracture.

Therapeutic massage of the shoulder joint for fractures has a number of advantages:

  • Carrying out a massage after bone fractures helps prevent tissue scarring;
  • it does not allow restriction of passive movements in the joint area;
  • massage for a fracture of the shoulder neck eliminates pain;
  • The effect of therapeutic massage is based on improving blood circulation and stimulating lymph outflow;
  • regular physiotherapeutic procedures ensure an active supply of nutrients to the area of the affected tissue;
  • promotes maximum cleansing of cells from waste products;
  • thanks to massage, the process of regeneration of bone and joint tissues is activated at the cellular level;
  • There is an acceleration in the process of merging damaged components. This allows you to fully restore the previous mobility of the joints in the injured arm as quickly as possible.

You should not self-medicate. All your actions must be coordinated with your doctor.

Is it possible to study independently or is it better to do it under supervision?

After healing the damaged bones and removing the plaster, the doctor must make individual recommendations for the development of the shoulder, elbow joint and the entire injured arm. Of course, we should not forget about the stress on a healthy limb. It is better to entrust a massage to a competent specialist. If it is difficult to find one, ask someone close to you to master the necessary complex; in extreme cases, perform the elements of the massage yourself.

Of course, the last option is the most difficult; the main thing to remember is that recovery and the absence of problems in the future depend on the systematic nature of the exercises and their correctness.

The same can be said about the complex of physical therapy: of course, classes under the supervision of a competent specialist are preferable, but, if necessary, you can carry them out independently, the main thing is that the load is selected optimally. After a fracture, many people unnecessarily spare the injured arm, while others put unnecessary strain. Both cause a lot of harm.

Recommendations for massage

In order for massage after a fracture of the shoulder neck to ensure rapid restoration and strengthening of bone tissue, it must be carried out taking into account important recommendations and rules. Typically, massage is performed by experienced specialists, but the patient himself should know the main features.

When performing a massage, be sure to consider the following recommendations:

  • if there is a bone fracture, massage must begin three days after removing the plaster cast;
  • limb massage can be carried out not only in a hospital setting, this procedure can also be done at home. To carry it out at home, you can have a special device - a massager. If this device is not available, then massage is best done with your hands;
  • During the first week, massage of healthy areas of the upper limb should be carried out;
  • at the initial stage, the intensity of movements should be kneading in nature;
  • at the first stage, physiotherapy is carried out only with the help of hands, for this you can use effective techniques - the first exercise is pressing on points with your hands, the second is stroking, the third is smooth rubbing with the pads of your fingers, the fourth is pincer-like pinching, the fifth is stretching with light percussive tapping;
  • each exercise needs to be done for 3 minutes, it should take 15 minutes in total;
  • subsequent massage procedures can be carried out longer; after a week, the time should be increased to 30 minutes;
  • with each session the strength and depth of impact increases;
  • To improve the effect, after 6 months you can use a special massage. You can also additionally use vacuum attachments or perform a massage procedure in vibration mode.

This massage is done from the fingertips and up to the shoulder. The intensity and number of procedures depends on the patient’s condition and the severity of the fracture.

The massage procedure should not cause pain or discomfort in the patient. For this reason, when carrying out it, you should listen to your feelings. If pain and other unpleasant feelings occur during a massage, it is better to make some adjustments and changes or cancel the procedure.

A set of exercises after a fracture of the tubercle of the humerus

To perform the exercises, you may need a stick; functions lost after a fracture can be easily restored with the help of a wrist expander or a small rubber ball. In the first stages, you often have to help yourself with your healthy hand.

Important: in the first stages of recovery, you should not be afraid of pain; if the exercises remain systematic, the pain will soon go away; If you don’t want to work out your injured arm, you can get significantly more problems.

Classes can be held in the swimming pool. Exercises in water are easier and more interesting, and swimming is one of the best rehabilitation procedures in the rehabilitation complex after a fracture of the greater tubercle of the humerus.

Exercise No. 1

Performed standing or sitting. Your arms should be extended, palms up, in front of you. Then they slowly bend at the elbows, while touching the shoulders.

Exercise No. 2

The arms slowly rise above the head through the sides, touching palms and lowering.

Exercise #3

You can place your hands on your belt or simply press them to your body with your elbows slightly bent. Perform slow rotations of your shoulders forward and backward.

Exercise #4

The arms are extended forward, then pulled back.

Exercise No. 5

Circular movements with hands. As a child, this exercise was called the mill.

Each exercise must be performed at least 5 times. Gradually, when the sore arm moves better, increase the amount - first to 7, then to 10 times. The complex does not take much time and is performed three times a day. You shouldn't do exercises right before bed.

Restrictions

Therapeutic massage for a fracture of the shoulder joint has some contraindications:

  • feverish conditions;
  • the presence of sharp pain in the shoulder area when moving or at rest;
  • if there are concomitant inflammatory processes with an acute course;
  • bleeding;
  • skin diseases, damage;
  • oncological processes;
  • the likelihood of blood clots.

For children, therapeutic massage is recommended to be carried out carefully. It should be done by an experienced massage therapist with medical education under the supervision of a traumatologist and physiotherapist. Each child needs an individual approach.

Before you start performing a massage, remember that you should not use strong blows, intense tapping, or tapping on the sore area during this procedure. The fact is that reflex tension of muscle tissue can cause shifting of bone components.

How to properly undergo rehabilitation after such a fracture

If you suspect such an injury, you should immediately contact a medical facility. Only a specialist can make the correct diagnosis and avoid worsening the situation. Contact a traumatologist immediately if:

  • after the injury, acute pain occurred in the shoulder joint, which intensifies with any movement of the shoulder, an attempt to abduct or raise the arm;
  • palpation of the damaged area causes increased pain, during examination a characteristic crunch of bone fragments is heard;
  • there may be hematomas, swelling is noticeable;
  • the person intuitively tries to protect the injured limb, the arm is bent and pressed to the body.

In this case, immobilization measures are taken to immobilize the shoulder. The arm is secured with a bandage or scarf; improvised means can be used as a splint; often the arm is tied to the body. If a person does not have allergies, you can give him a painkiller.

Important: urgently call an ambulance or help the victim get to a medical facility, since only a doctor can determine the extent of the damage.

The diagnosis is made on the basis of radiography; a more accurate modern method is computed tomography or magnetic resonance imaging (CT, MRI).

For a normal fracture, a plaster cast is applied, which ensures immobility of the shoulder joint and allows the damaged bones to heal and the muscles to recover. If the injury results in a comminuted fracture, displacement, or severe damage to the joint, surgery is necessary to fully restore the function of the arm. Surgeries are not performed on older patients; the reason is the body’s reduced regenerative abilities.

Physiotherapy

It is recommended to begin performing therapeutic exercises from the first days of a humerus fracture, while the plaster cast has not yet been removed.

Therapeutic exercises should be carried out taking into account the following recommendations:

  • after the fracture, you can begin to move your fingers;
  • You should not perform actions that cause pain;
  • after a week you can begin to tense the muscles in the shoulder area. This must be done so that the joint is stationary;
  • exercise for muscle tension is required to be done 10 approaches at a time. There should be a minimum of 6 tensions in each approach;
  • the same exercise should be done for a healthy arm; it also needs to maintain muscle tone;
  • After removing the cast, you can move on to active movements in the area of the shoulder and elbow joint.

Why is rehabilitation needed?

Restorative treatment is prescribed simultaneously with the main one and is aimed at preserving the motor function of the limb. It includes several activities, including massage. The purpose of this method is to prevent the development of muscle contractures and limitation of movements in the future.
